To update the credit card you'd like us to use for any balances, please fill out this Credit Card on File form. Click here and then choose QuickPay at the bottom right to make a one-time payment. You’ll need either your account or statement number from your bill which you should hav received through the Elation Portal (see above). If you can't call in, there are two ways to pay online: We require a functioning credit or debit card on file at all times. Please call us to update your payment card to keep on file. You can also see a list of your current medications, upcoming appointments, and most recent treatment recommendations. All billing statements will be sent to your account and you can send us non-urgent messages through the portal messaging service. We ask that all patients set up an account through the Passport Patient Portal for our medical record system, Elation. You can only set up a working spruce account if you are already a patient at The Via Center. Click here to set up your app and account.
